In-Home Services

Five County contracts with the Division of Aging & Adult Services for the administration of three in-home service programs which are funded by State and Federal dollars.  We are responsible for the administration, eligibility and case management of these programs and sub-contract with local providers for the direct services.   We are also a Medicaid provider with the Utah Department of Health to provide the case management services for New Choices Waiver Program which provides home and community based care for seniors and disabled adults transition from a nursing facility.

AGING WAIVER

Aging Waiver is designed to provide services to help frail, elderly individuals receive the services they need to remain in their own homes as an alternative to care in a nursing facility.

The Program is supported through a joint federal and state funded program call Medicaid 1915(c) Home & Community Based Service Waivers; with administrative oversight through the Utah Department of Health & Division of Aging & Adult Services.

A referral can be made to our office by the individual, family member, health care provider, etc. Once a referral is received, a care manager will contact the individual referred and/or family member to discuss the programs and available services.  If the individual so chooses, a demographic intake and screening will be completed over the phone to identify potential need and pre-screen for program eligibility.
