The Area Agency on Aging-Five County Association of Governments has been providing services to frail elderly and disabled adults for over 20 years. We have an experienced staff with extensive skills and knowledge of community resources who can access these services to meet the needs of the aging population across five counties - Beaver, Garfield, Iron, Kane, and Washington - in Southern Utah.

Coming together from a variety of backgrounds and experiences, our skilled team of professionals are the backbone of Area Agency on Aging - Five County AOG. Their ideas help shape the direction and mission of our organization as it continues to develop. Read on to learn more about some of our incredible team members.

Carrie Schonlaw

Deputy Director of Aging and Human Services

Carrie received a BS in Psychology from Southern Utah University and is a licensed Social Service Worker. She started her career at Five County in 1997. She has extensive experience and expertise in the field of expanding Aging Services through local, State and Federal funds, innovative grants, and program development.

Tracy HeavyRunner

Home and Community Based Program Director

Tracy has a BS Degree in Psychology from Southern Utah University. She has worked at Five County since 1996 She is a licensed Social Service Worker.  She is a RCI Coach and a Master Trainer for Chronic Disease  Self Management program as well as Master Trainer for Diabetes  and Pain self Management.

With us since our founding, Alex Smith is one of our veteran team members. They bring their years of experience and skills to the forefront in helping our organization grow and succeed.

Aiden Johnson joined Area Agency on Aging - Five County AOG with the vision and drive to improve upon our Public Services Organization, and focused on developing the organization with experience and integrity.

Becky McRae

Case Manager

Becky has her BS degree in Sociology from Southern Utah University. She is a licensed Social Service Worker.   She has worked with the Aging population since 2007  and has been with Five County since 2013. She is the Case Manager for the Veterans Services Directed Home Base Services, New Choice Waiver, Alternatives and Caregiver programs. She enjoys being outdoors on the hiking trials of Southern Utah.

Donna Sutherland

Registered Nurse

Donna has been a RN for 40 years. She has worked in  hospital settings, Home health care settings, and Rehabilitation  settings. She has been with Five County for 2.5 years in our Aging Waiver Program.

Heather Cox

Case Manager

Heather has  BS in Sociology.  She is a licensed Social Service Worker. She has been with Five Counties since 2013.  She works in Iron, Beaver and Washington Counties as a Case Manager for the Aging Waiver, Alternatives and Caregiver Programs. She is a certified trainer for Dealing with Dementia, RCI REACH, Dementia Dialogues, and Caregiver Academy. She facilitates monthly support groups in Cedar City. She enjoys spending time with her family enjoying outdoors activities and designing and knitting her own patters.

Janeil Esplin

Case Manager
Veteran Service Officer

Janeil has her BS degree in Psychology. She is a licensed Social Service Worker.  She has been with Five Counties since 1999. In 2016 Janeil gained her Veteran Service Officer Accreditation in order to assist Veterans 60 years+ and surviving spouses navigate the VA health and benefit programs. She travels throughout all Five Counties to provide this service. She also works in the New Choice Waiver Program. She enjoys hiking, cooking , jogging, traveling, learning and gardening.

Joni Lyman

Foster Grandparent and Senior Companion Program Coordinator

Joni has been with Five Counties since 2016. 
She finds, recruits, trains and works with Volunteers willing to serve their community. 
Joni loves living in Southern Utah, she loves the outdoor opportunities. Some of her favorite things to do are playing pickle ball and reading.

Kristi Lasson

Case Manager

Kristi obtained her BS degree in Family Human Development, with an emphasis in Child Development from Utah State University. She is a licensed Social Service Worker, and has been with Five Counties since 2000. She works with The New Choice Waiver, Alternatives and Caregiver Programs. Kristi is a Certified trainer for Dementia Dialogues. Kristi loves the sunshine and loves raising her two daughters here in St. George

Lisa Anderson

Long Term Care Ombudsman

Lisa has a BS Degree of Social Science with major in Psychology and minor in Political Science. She is a licensed Social Service Worker . She has been with Five Counties since 2010

Mike Hart

Retired Senior Volunteer Program Coordinator

Mike has been in non-profit management for 10 years and works almost exclusively with volunteers. He travels to all Five Counties; recruiting, training, encouraging and assisting trainers in facilitating their classes. He enjoys swimming, running, anything outdoors and genealogy.

Nancy Morrill

Case Manager

Nancy has her BS in Psychology from University of Utah. She is a licensed Social Service Worker and is a National Certified Dementia Practitioner. She has  been with Five Counties since 2013  working in New Choice Waiver, Alternatives, and National Caregiver Programs. She covers Washington, Garfield and Kane Counties. She is a Certified trainer for Dementia Dialogues, RCI REACH, Dealing with Dementia, and Caregiver Academy, and facilitates  a support group in Kane County.
