In Home Services Case Management

Helping Older Adults to Remain Safe at Home

Home and Community Based Services (HCBS) are State and Federal programs designed to help seniors with limited resources. The goal of Aging programs is to assist seniors in their own home as long as their needs can be safely met. Five County Area Agency on Aging currently provides case management for four programs: Medicaid's Aging Waiver, State Alternative Program, Veteran's Self Directed, & National Caregiver Program.

Aging Waiver

Medicaid program designed to help older adults live independently.

Alternatives Program

State program that coordinates services to assist low-income older adults living at home.

Veteran Directed Care Program

Provides Veterans with opportunities to direct care services to continue living safely at home.

National Caregiver Support Program

Provides support and education to caregivers, particulary those who care for a loved one with dementia.
